JOINING THE PRACTICE

New residents to the area are welcome to register.

If you do not have a medical card you will be asked to complete a registration form giving your name address date of birth and details of your previous address and doctor. This allows the Primary Care Trust to arrange for your medical record to be transferred to us.

If you do have a medical card, please bring it with you to the surgery when you want to register as this shows all the information required.

When you register with the practice you will be invited to complete a new patient questionnaire. This allows any significant health problems or family history to be recorded prior to the receipt of your medical record. If you take regular medication please see a doctor before you run out of medicines.

You will also be given a practice leaflet, which gives you all the information about the services provided by the practice and the surgery opening times.

If you are in the area for a short time and require medical treatment you are able to register with this practice as a temporary resident. This gives you the same access to general medical services as registered patients.
If you are going to be in the area for longer than three months you should register fully.
