We expect you to treat the doctor’s
and staff with the same respect and courtesy that we
show to you. You have a responsibility to attend appointments
that you have made or to notify us if you are unable
to attend.
You should only request a home visit
if unable to attend surgery for medical reasons.
Misuse of the services we provide will
ultimately be to the detriment of all in our community..
You should notify us promptly of any
changes in your personal details, especially of a change
in telephone number.
You can expect to be offered explanations
and information about your health, illnesses and treatments.
Access to Medical Records
You are entitled to access your clinical
records or any other personal information held about
you. Please ask at Reception for an “Application
for Access to Medical Records” form.
There is no charge to view
your records if they have been updated in the last
40 days.
There is a charge of £10
to view your records if they have not been updated
in the last 40 days.
If you want a copy of the health records,
there is a charge of 20p per copied sheet.
The maximum fee will depend on how the records are
stored:
on computer: maximum £10
partly on computer and partly
in another form: maximum £50
totally in another form:
maximum £50
Confidentiality and Data
Protection
Confidentiality is observed by all members
of the practice at all times. All patient consultations
and records are confidential and all staff are fully
trained in confidentiality.
The practice is fully registered under
the Data Protection Act. Information in your medical
records is kept confidential at all times and only
disclosed to others for purposes related to your healthcare
(except where you have given permission). However
there may be occasions when information may be used
for audits and preparing statistics on performance
and activity (in which case steps are taken to ensure
patients are not identified). If you would prefer
for your information not to be used in this way, please
notify the Practice Manager.
Data is held about you on paper and in
computer records. The purpose for which this data is
held is the provision of patient care in NHS hospitals
or community services, family practice or in private
health care institutions.
